What is Total Triage


·        Total Triage or Patient Triage is a simple online form that you complete via our website www.sheerwaterhealthcentre.nhs.uk


·        The forms are for medical issues/prescription requests & admin queries


·        Our clinical team will review your request and assess your clinical need to decide on the best course of action for you. This could be an urgent on the day appointment/routine appointment/telephone call/request for more information, or you could be directed to another appropriate service


·        Please only submit one medical issue per form and put in as much information as you can so the GP who is triaging your request can respond to you appropriately & promptly


·        The forms are open Monday to Friday 8am – 4pm please allow 48 hours for a response. Once the forms are closed and you require urgent help please call the surgery on 01932 343524 and the reception will be able to assist you


·        Our Practice phone lines will remain open for anyone with accessibility needs who might find it difficult to complete this form and a member of our Patient Support team will complete it with you.


·        If English is not your first language our Patient Support Team can use an official translation service on the phone
